Home Health Speech Therapist

About the Role:

We are seeking a skilled and compassionate Speech Therapist to fill this role. The ideal candidate will have a strong background in home health care, an understanding of state and federal regulations, and a passion for helping patients live their best lives.

Key Responsibilities:
  • Conduct assessments to evaluate patients' speech and communication skills
  • Develop treatment plans in collaboration with patients, families, and caregivers
  • Provide speech and language therapy to patients in their homes
  • Monitor and document patient progress towards treatment goals
  • Educate patients, families, and caregivers on speech therapy techniques and strategies
  • Participate in interdisciplinary meetings to discuss patient progress and treatment plans
  • Maintain accurate and complete patient records in accordance with policies and procedures
Requirements:
  • Bachelor's or Master's degree in Speech-Language Pathology
  • Current license to practice as a Speech Therapist in the state
  • Minimum of 1 year of experience working with patients in a home care setting
  • Strong verbal and written communication skills
  • Ability to work independently and as part of a team
  • Excellent time management and organizational skills
  • Ability to travel to patient homes
