About the Role

The Primary Healthcare Provider we are seeking will be responsible for delivering exceptional patient care through comprehensive assessments, diagnosis, and treatment planning.

Key Accountabilities

  • Evaluate patient health status through interviews, physical examinations, and review of medical histories.
  • Order and interpret diagnostic tests, such as laboratory tests and radiology studies, to inform treatment decisions.
  • Develop and implement individualized treatment plans in collaboration with physicians.
  • Prescribe medications based on the scope of practice allowed under the collaborative agreement with the supervising physician.
  • Document patient care activities accurately and efficiently.
  • Maintain open communication with patients, families, and healthcare teams to ensure continuity of care.

Requirements

  • Hold a Master's Degree in Nursing from an accredited institution.
  • Possess certification as a nurse practitioner from a recognized certifying agency.
  • Meet the licensure requirements for practicing nursing in the State of Illinois.
  • Have a minimum of one year of experience as a nurse practitioner or a related field.
