Key Responsibilities:- Support patient access to essential health services
- Work closely with clinicians, enrolled nurses, and internal/external stakeholders
- Liaise with clients, carers, service providers, and referrers across diverse cultural and socioeconomic backgrounds
- Coordinate care and support for clients and their networks across the community
- Contribute to an inclusive, responsive, and patient-focused healthcare experience
- Current Authority to Practice as a Registered Nurse with AHPRA
- Demonstrated recent experience as a Registered Nurse in acute or community care settings, with well-developed clinical knowledge and ability to provide safe, high-quality, patient-centred care
- Ability to communicate and engage effectively with consumers, carers, and multidisciplinary healthcare teams to identify care needs and ensure timely, person-centred responses
- Proven ability to apply critical thinking and clinical judgement in managing complex patient care, ensuring delivery of safe, evidence-based nursing practices
- Australian C-Class driver's license (including P1 or P2)
