We are seeking a highly skilled Professional Case Manager - Homestay Program Coordinator to join our team in Townsville, QLD. In this role, you will provide mobile, outreach, and centre-based case management support to individuals and families experiencing vulnerabilities in the community.

Job Description

As a Professional Case Manager - Homestay Program Coordinator, you will operate from a strengths-based and capacity-building framework to ensure the promotion of social inclusion, human rights, participation, and consumer choice. You will collaborate with other agencies and organizations to source appropriate support for clients.

Key Responsibilities

  • Provide high-quality, specialist casework support and case management to individuals and families experiencing homelessness or at risk of homelessness.
  • Holistically assess complex client needs, including conducting risk assessments and developing effective case plans and safety planning.
  • Work competently with staff and clients from diverse backgrounds with respect and humility.
  • Collaborate with internal and external stakeholders to contribute to interagency collaboration and lead collaboration, advocacy, and networking efforts.

Requirements

  • A Degree or Diploma in Social Work or Social and Community Services with a minimum of 1 year post-graduate experience; or other relevant qualifications, skills, and experience that demonstrate equivalence to this standard.
  • Proven ability to work effectively in a fast-paced environment with multiple priorities and deadlines.
  • Demonstrated understanding of how to promote social inclusion, human rights, participation, and consumer choice.
  • Experience working with children, adults, and families from diverse cultural and linguistic backgrounds.
