Chronic Disease Management Opportunity

We are seeking a skilled and dedicated professional to join our team as a Chronic Disease Management Specialist. This is an exciting opportunity to make a real difference in the lives of people living with chronic disease.

About the Role

  • Coordinate services for chronic disease management according to the Latrobe Chronic Disease Action Plan 2022 – 2027.
  • Promote and support early detection and intervention activities.
  • Communicate evidenced-based screening options across the network.
  • Expand health professional resources and develop technology to support self-management.
  • Build capacity between community-based services and clients/carers.

Requirements

  • Registered Nurse with condition-free AHPRA registration.
  • Experience in Chronic Disease Management.
  • Excellent interpersonal skills and the ability to work well as part of a team.
