The successful candidate will have expertise in mental health services, Clozapine management, and care coordination. They will play a critical role in providing patient-centered care, working collaboratively with healthcare professionals to ensure the best outcomes for patients.
The role involves providing care in community settings, homes, or clinics, supporting individuals of all ages with complex mental health needs, including co-existing physical conditions, substance use, and social challenges. The ideal candidate will be able to develop and implement personalized care plans, manage medications, and ensure a safe environment for patients.
**Key Responsibilities:**
- Provide high-quality nursing care to patients in community or inpatient settings.
- Administer and manage Clozapine treatment, including monitoring patients for side effects and adjusting care plans as necessary.
- Act as a care coordinator for patients with complex mental health needs, working with multidisciplinary teams to develop and implement individualized care plans.
- Conduct comprehensive mental health assessments and participate in treatment planning and review processes.
- Advocate for patients and ensure that care is culturally sensitive and aligned with best practices.
Requirements:
- Unrestricted AHPRA registration.
- Full working rights in Australia.
- Credentialed Bachelor of Nursing Degree with mental health qualifications.
- NSW WWCC (or willingness to obtain).
- Minimum 2 years postgraduate experience.
