Job Opportunity:

We are seeking an experienced and compassionate healthcare professional to take on a leadership role in delivering person-centred care to patients with complex needs.

The Nurse Navigator – Palliative Care will work across sectors to coordinate holistic, high-quality care during some of life's most vulnerable moments. This is a unique opportunity to make a real difference in the lives of our patients and their families.

Key Responsibilities:
  • To provide expert level healthcare coordination to patients with complex health conditions requiring proactive and integrated approaches.
  • To work across system boundaries to actively advocate and improve health literacy, in close partnership with patients and families, other health and allied-health services.
Requirements:
  • Advanced clinical and professional practice capabilities to deliver a positive patient centred health experience for individuals.
  • The ability to facilitate a safe and efficient patient journey across the care continuum through the reduction of service fragmentation, educating and empowering individuals and coordinating patient care.
