The Client Care Manager is responsible for coordinating and delivering high-quality care services to clients in their homes. This role involves building strong relationships with clients and their families, conducting comprehensive assessments, and developing personalized care plans.
Key Responsibilities:- Develop and implement effective care plans that meet the individual needs of clients
- Build trust and rapport with clients and their families through regular communication and support
- Collaborate with healthcare professionals and other stakeholders to ensure seamless care delivery
- Maintain accurate and up-to-date client records and information
- Formal tertiary qualification in nursing or a related field
- Current registration as an Enrolled or Registered Nurse with AHPRA
- Demonstrated knowledge and experience in aged care
- National police check and industry-applicable licenses
- Valid First Aid and CPR certificate
