About the Role

The Coordinator Pause Program will be responsible for developing person-centred care plans and working closely with clients and carers to deliver flexible respite options. This role is a pilot initiative aimed at enhancing the wellbeing of both family carers and people living with dementia.

The ideal candidate will have experience working with people living with dementia and their carers, along with a strong understanding of the challenges they face. They will be a confident communicator with well-developed interpersonal, record-keeping, and computer skills.

The successful applicant will work 75 hours per fortnight in a part-time role, with flexibility on which weekdays are worked. Key Responsibilities
  • Program Development and Implementation: Support the Manager in implementing the Pause Program with clients and carers. Engage with participants to provide timely, person-centred dementia respite support. Identify and coordinate informal and formal respite options tailored to individual needs.
  • Communication and Teamwork: Work with clients, carers, and stakeholders to ensure coordinated care. Contribute to team meetings and provide program updates as required.
  • Operations, Administration and Documentation: Maintain accurate documentation in line with procedures and standards. Use electronic systems to manage data and report as per funding requirements.
