Job Summary

We are seeking a qualified and experienced Registered Nurse to join our team as a Clinical Care Coordinator. As a key member of our team, you will be responsible for delivering clinical care and supervision to our clients in their homes. You will work closely with our clients, their families, and our care staff to ensure that our clients receive the highest level of care and support.

Key Responsibilities

  • Deliver high-quality clinical care to clients in their homes, including assessments, care planning, and medication management.
  • Supervise and manage a team of care staff, providing guidance and support to ensure that our clients receive the best possible care.
  • Process intakes, conduct client care assessments, and develop care plans that meet the individual needs of our clients.
  • Coordinate the provision of quality person-centred care, ensuring that our clients receive the best possible outcomes.
  • Manage care staff schedules, appointments, and in-home services to ensure that our clients receive timely and effective care.
  • Conduct general administration tasks, including data entry and reporting, to ensure that our operations run smoothly.

Requirements

  • Qualified Registered Nurse with current AHPRA registration.
  • Experience in assessing and planning care needs within a community setting.
  • Valid Driver's Licence and reliable, presentable vehicle.
  • Exceptional communication and interpersonal skills.
  • Ability to work autonomously and as part of a team.
  • Commitment to ongoing professional development.
