**Job Description:**

We are seeking a highly skilled Mental Health Community Liaison to join our team in a rural community setting. As a key member of our multidisciplinary team, you will play a vital role in providing welcoming and professional access to mental health supports.

The successful candidate will have the opportunity to make a real difference in the lives of individuals and families in our community. You will work closely with our Centre Manager/Clinical Lead to implement our service delivery model, including assessment pathways and transition of care arrangements.

You will be responsible for working as part of a multi-disciplinary team to provide integrated, evidence-based mental health services. This will involve supporting clients with complex needs, developing strong relationships with local healthcare providers, and contributing to the effective operation of our centre.

To be successful in this role, you will ideally hold a tertiary qualification in an Allied Mental Health discipline and/or have relevant experience working in Mental Health service delivery. You will also require excellent communication skills, experience working with people from diverse backgrounds, and a current driver's licence.

Key Responsibilities:

  • Support the implementation of our service delivery model, including assessment pathways and transition of care arrangements.
  • Work as part of a multi-disciplinary team to provide integrated, evidence-based mental health services.
  • Develop strong relationships with local healthcare providers to ensure seamless care transitions.
  • Maintain accurate records and contribute to quality improvement initiatives.

About You:

  • Tertiary qualification in an Allied Mental Health discipline or relevant experience working in Mental Health service delivery.
  • Excellent communication skills and experience working with people from diverse backgrounds.
  • Current driver's licence.
