We are seeking a skilled Care Coordinator to join our team. As a key member of our staff, you will be responsible for delivering high-quality care to our clients in a community setting.
Your main focus will be on meeting with clients, assessing their needs, and handling various requests, enquiries, or complaints. You will also be required to prepare in-home care assessments and care plans to meet client requirements.
To ensure accurate and comprehensive documentation, you will need to maintain up-to-date records in our databases and software programs. This includes adhering to organisation policies and legislative frameworks.
Key Responsibilities:- Client meetings and needs assessment
- In-home care assessments and care planning
- Accurate documentation in Right at Home databases and software programs
- Current registration with AHPRA
- Community nursing experience is essential
- Australian National Police check and Working With Children's Check are required
- First aid and CPR certificate, as well as a driver's licence and reliable car, are necessary
