We are seeking a highly skilled Registered Nurse to join our team as a Geriatric Care Specialist. In this role, you will work closely with our hospital home teams and Aged Services to ensure the provision of integrated care between acute and sub/non-acute sectors.
Key Responsibilities- Undertake comprehensive geriatric assessments to identify early discharge issues and implement discharge planning processes for complex patients.
- Act as a resource for clinical staff in relation to changed behaviors associated with delirium or other cognitive impairments such as dementia.
- Undertake specific program activities for AST to ensure quality and safe clinical care processes of elderly patients.
- Work as part of a dynamic, fast-paced, supported team in a multi-disciplinary work environment.
- Collaborate with other healthcare professionals to provide comprehensive care to patients with complex needs.
- Registered with the Nursing and Midwifery Board of Australia as a Registered Nurse.
- Current Driver's Licence.
- Proven experience in the comprehensive assessment and management of clients with aged care related issues.
- Experience with managing clients with delirium and/or dementia.
- A competitive salary range of $75,902 - $102,908 (pro-rata) per annum.
- 11.5% superannuation contribution.
- Access to salary packaging benefits.
- Professional Development Allowance of up to $500 per annum.
- Post Graduate Allowance of up to 10% based on qualification type.
