About the Role

We are seeking a highly motivated and person-oriented Social Worker to join our team at Amana Living's Mosman Park Care Centre. As a Social Worker, you will play a crucial role in promoting the psychological safety and well-being of our clients/residents and their representatives. This dynamic role requires a strong understanding of the Aged Care sector and the ability to provide vital information and support tailored to each individual's complex care and social circumstances.

Key Responsibilities
  • Undertake social work assessments, including psychosocial assessments for all assigned clients in consultation with other team members
  • Liaise with clinical staff/pharmacy and inform them of residents' planned discharge dates for transport arrangements
  • Case management may include supporting clients with State Administrative Tribunal (SAT) and guardianship processes
  • Plan and undertake social work interventions with clients, in conjunction with their families and the multidisciplinary team
  • Provide care coordination, counselling, advocacy, mediation, referrals, and support for clients when transferring them to either their home environment or a Residential Care Facility
  • Provide consultation and support to Nursing and Allied Health professionals regarding psychosocial aspects of care for clients
  • Ensure compliance is maintained in line with Amana Living's policies and procedures, and accreditation standards are sustained
Requirements
  • Bachelor of Social Work
  • Knowledge of the Aged Care Sector
  • Strong time management skills and ability to prioritise tasks efficiently
  • Excellent problem-solving and negotiation skills
  • High attention to detail
  • Exceptional verbal, written, and interpersonal communication skills
  • Evidence of COVID-19 and current Flu vaccinations per Amana Living policies
  • Ability to obtain a National Police Clearance (within 6-months validity)
  • Successful completion of pre-employment medical form and reference checks
