Primary Care Services Provider Opportunity
">As a skilled Nurse Practitioner, you will be responsible for delivering comprehensive primary care services to a defined panel of patients. Your expertise in medical knowledge and exceptional interpersonal and leadership skills will enable you to provide high-quality patient care.
This role requires the ability to deliver evidence-based health care, manage patient panels effectively, and utilize research and evidence-based practice to evaluate patient care delivery systems.
The key responsibilities of this position include conducting clinical examinations, diagnosing and treating acute and chronic diseases, providing preventive care, and coordinating care for a panel of primary care patients.
In addition to these duties, you will be expected to monitor PACT teamlet performance, lead interdisciplinary teams, and acquire knowledge and skills to develop expertise in your area of practice.
To be successful in this role, you must be able to exercise medical decision-making, provide a broad range of diagnostic and therapeutic services for a specific group of patients, and elicit cooperation from others.
A minimum of two years of experience in primary care is preferred.
Advanced Practice Nurses (NPs) must possess and maintain an active, current, full, and unrestricted APN license to practice as an NP in a State, Territory or Commonwealth of the United States, or in the District of Columbia.
Basic Certification is also required, with ANCC, AANPCB, or other nationally recognized certifying bodies being acceptable.
Applicants pending completion of educational or certification/licensure requirements may be referred but not hired until all requirements are met.
