We are seeking a highly skilled and experienced Senior Transition Specialist to join our team at The Busy Group. As a key member of our team, you will play a critical role in providing one-to-one support to ADF Members with significant complex medical conditions to transition from the ADF to civilian employment or meaningful engagement.
Key Responsibilities- Engage in Practices to Build Resilience
Engage in practices to build a member's resilience, rehabilitation, and an enduring capability to continue to reach their future wellbeing goals once transition support has concluded.
Act as a Triage PointAct as a triage point for new referrals, ensuring to review incoming referrals and make recommendations to team leaders of their status being, low, medium, or high complexity.
Support Team GoalsSupport team goals and provide guidance and mentoring to other Transition 4 Meaningful Engagement specialists in order to ensure high-quality vocational support is provided to members.
Assist Leadership TeamAssist the leadership team to complete caseload audits by providing clinical oversight and recommendations for improved practice, that can be provided to the team.
Complete Case Management DutiesComplete case management duties to ADF Members who are considered highly complex, including but not limited to:
- Work one-on-one with the member to meet the program's objectives and outcomes.
- Maintain minimum contact schedule.
- Complete case management administration duties such as file noting, updating profile records, and completing reporting requirements.
- Participate in the coordination and delivery of Transition Seminars; Participate in Seminars regularly throughout the year, promoting transition services in front of a public audience of defence personnel.

The Busy Group is a not-for-profit organisation which presents 45 years of experience as a community partner. With 5 business entities operating across 100+ locations Australia-wide, we offer an extensive range of employment services and programs, vocational training, apprenticeships, mentoring, NDIS, disability employment support, specialist schools, and allied health services.
About You- Experience working with vulnerable cohorts in a general capacity within the health, employment, education, or disability sector
- Demonstrated understanding of evidence-based best practice standards for a range of presenting issues
- Proven capacity and ability to work with professional integrity and support vulnerable people
- Demonstrated understanding of contextual ethical reasoning practices
- Knowledge and understanding of the support needs of vulnerable people
- Capacity to support clients to reach broader biopsychosocial goals
- Knowledge and understanding of current mental health issues in Australia, including impacts on client presentations and needs
- Highly desired:
- Rehab Counselling qualification and experience, with Registration to Rehabilitation Counselling Association of Australasia OR
- Degree qualified Social Work qualification with registration through Australian Association of Social Workers (AASW)
- Mandatory qualifications:
- Counselling, Psychology, Social Work, or Rehab Counselling and ability to be registered through a professional association
- Baseline Security Clearance Or Ability to obtain clearance
- State Specific Working With Children/Vulnerable people check.
- National Police Check
- Open drivers' licence
