Job Title: Occupational Therapist

Alfred Health is seeking a skilled Occupational Therapist to join our Adult Health team at Caulfield Community Health Service.

Job Summary

We are looking for a highly motivated and experienced Occupational Therapist to provide comprehensive assessments, recommendations, and ongoing OT services to clients living at home. The successful candidate will work within a multi-disciplinary team to ensure a holistic approach to client support and liaise with relevant agencies to coordinate, facilitate, and communicate client treatment and outcomes.

Key Responsibilities
  • Provide comprehensive assessments, recommendations, and ongoing OT services to clients living at home.
  • Develop and implement person-centered care plans.
  • Work within a multi-disciplinary team to ensure a holistic approach to client support.
  • Liaise with relevant agencies to coordinate, facilitate, and communicate client treatment and outcomes.
  • Demonstrate an understanding of pathways within the health care system to help facilitate appropriate care and advocate for clients with complex care needs.
Requirements
  • Degree in Occupational Therapy.
  • Registration with the Occupational Therapy Board of Australia and adherence to continuing professional development requirements.
  • Registration with the State-wide Equipment Program (SWEP).
  • At least two years of experience, including demonstrated occupational therapy clinical experience relevant to the community health sector.
  • Experience in home modifications and equipment prescription.
  • Current Victorian driver's license.
