Home Care Enrolled Nurse

Empower Healthcare is seeking a passionate and dedicated Enrolled Nurse to join our team in providing exceptional care to clients in their homes. As a District Nurse with our organization, you will be responsible for delivering comprehensive services across home care clients in the Northern & Western Suburbs of Melbourne.
Key Responsibilities:- Conduct thorough clinical assessments and develop tailored care plans to meet the unique needs of each client.
- Provide a range of nursing services, including medication management, wound care, diabetes management, and cognitive assessments, to ensure optimal client outcomes.
- Collaborate with other healthcare professionals to provide a multidisciplinary approach to client care, promoting holistic well-being and independence.
- Educate clients and their families to empower them to manage their health effectively and live safely within their communities.
- Work closely with Case Managers and NDIS Support Coordinators to ensure seamless coordination of care and achieve positive client outcomes.
- Unrestricted Registration with AHPRA as an Enrolled Nurse.
- Availability between Monday-Friday 8AM-4PM.
- Sound clinical knowledge and skills, with experience working with clients who have chronic and complex health needs.
- Excellent verbal and written communication skills, with the ability to work effectively with diverse clients and their families.
- Excellent time-management skills and attention to detail, with the ability to prioritize tasks and complete documentation to a high standard.
