Community Physiotherapist

This is a temporary role for a Physiotherapist within our community-based model in South/Western Sydney.
The position involves delivering physiotherapy services to home care, NDIS and Residential Aged Care clients.
The Role- Conduct comprehensive assessments and deliver tailored physiotherapy treatment plans for clients in their own homes through home care packages/NDIS.
- Work collaboratively with other allied health professionals to provide multidisciplinary care plans.
- Educate clients and their families on how to optimize their ability to live healthy lives within the community.
- Establish effective communication with service providers.
- Work independently to prioritize tasks, maintain records, and provide patient reports.
