About the Role

Pink Hope is seeking a skilled Nurse Navigator to join our team in supporting individuals in managing their cancer risk. As a key member of our organization, you will play a vital role in empowering Australians to understand and reduce their risk of hereditary cancer.

Key Responsibilities
  • Assist patients in navigating the healthcare system and developing a holistic care plan to manage cancer risk.
  • Support the medical review of content and campaigns to ensure evidence-based and up-to-date guidelines.
  • Participate in network meetings and quality improvement initiatives to drive excellence in patient care.
  • Engage in nurse education programs to enhance clinical knowledge and skills.
  • Develop and deliver online education sessions on relevant topics for high-risk communities.
  • Contribute to data collection, health outcomes reporting, and programmatic evaluation to inform service improvement.
  • Evaluate clinical care, resource utilization, and the development of new clinical tools, forms, and procedures.
  • Perform administrative tasks and small projects to support patient populations, including developing information materials and responding to support group questions.
Requirements
  • Expertise in hereditary cancer, breast/ovarian cancer, and the Australian health system.
  • Proficient computer skills, including Microsoft Office (Word and Excel).
  • Minimum 5 years of experience in a clinical setting.
  • Self-disciplined, energetic, passionate, and innovative with excellent communication skills.
  • Confident in presenting at educational events and webinars.
  • A team player with the ability to follow systems and protocols to achieve common goals.
  • Highly organized with sound judgment, decision-making, and problem-solving skills.
  • Able to maintain confidentiality in accordance with practice and local legislation.
  • Graduation from an accredited university with a Bachelor of Nursing Degree and AHRPA registration.
