Lead Community Care Specialist

The role of Lead Community Care Specialist is to oversee the delivery of high-quality clinical and care services in a specific region. This position will be based at a central office, coordinating resources and activities to achieve person-centered care for clients.

Key Responsibilities:

  • Ensuring all clients have a current and holistic assessment and service plan that manages their assessed needs within their budget and with a health-promoting focus.
  • Actively engaging with the community to promote the organization as a provider of choice for home-based care services to older adults.
  • Advocating for clients to assist with making informed decisions.
  • Monitoring the quality of comprehensive care planning strategies to ensure clients' quality of life and independence are optimized, while managing risks and duty of care.
  • Ensuring all assessments and documentation meet legislative requirements.

Requirements:

  • Bachelor's degree in Nursing or equivalent qualification and current registration with AHPRA as a Registered Nurse.
  • Current First Aid Certificate and Current CPR Certificate.
  • Unrestricted driver's license and own registered and insured vehicle.
  • Strong organizational and time management skills.
  • Excellent interpersonal and communication skills, with ability to build positive relationships with clients and their representatives.
  • Effective decision-making ability, including identifying potential risks and escalating as appropriate.
  • Proficient computer and keyboard skills, including use of aged care systems and processes.
