We are seeking a dedicated professional to support individuals aged, frail, or with disabilities under the Consumer Directed Care model in our region.
You will provide personalized services such as personal care, medication support, cleaning, meal preparation, transport, and social support. You'll work closely with consumers, respecting their choices and goals, as they actively manage their care and decide the services they receive.
Key Responsibilities:- Provide personal care and support to individuals with varying needs and abilities.
- Assist with daily tasks such as cleaning, meal preparation, and transport.
- Support individuals to maintain independence and engage in community activities.
- Work collaboratively with consumers, families, and other healthcare professionals to achieve desired outcomes.
- Completed Certificate III or higher in Individual Support (Aged Care, Disability, or Home and Community).
- Proven experience in exercising own judgment and initiative in the day-to-day execution of a position.
- Experience in providing direct care services in a health-related field.
- Experience in working with aged, frail, or disabled persons.
- Proficient in using computer packages such as Microsoft Word and Excel.
We provide a wide range of services across 337,626 square kilometres, taking in the Eyre Peninsula, western part of South Australia, and north to Coober Pedy.
Our services include acute care, medical, accident and emergency, surgery, birthing and midwifery, specialist consultancy, renal dialysis, chemotherapy, transfusions, rehabilitation, residential and community aged care, respite care, transitional care packages, Aboriginal health, mental health, allied health, community health, community nursing, palliative care, community home support packages, and home modifications.
