About the Role

Aveo is seeking a skilled and compassionate Clinical Case Manager to join our team at Rochedale Gardens Retirement Living. As a key member of our care team, you will be responsible for assessing the care needs of our residents, providing support and guidance to access government-funded services, and managing home care packages.

Key Responsibilities

  • Meet with prospective residents and their families to understand and assess their care needs
  • Provide tailored information on care needs, services, and funding options available
  • Establish home care plans aligned to resident care needs, ensuring supports are in place as required
  • Prepare and maintain records, documents, and reports as required
  • Deliver excellent customer service to residents, families, staff, management, visitors, and external health professionals
  • Monitor the effectiveness of services by ensuring consumers receive the care and support services they require to meet identified needs, goals, and preferences
  • Liaise effectively with external service providers who provide care within the community
  • Promote Aveo Care at Home services by attending community events, assisting with the transfer of home care packages, and providing support throughout the My Aged Care journey

Requirements

  • Current AHPRA Div 1 registration with experience preferably in a Home or Community Care setting
  • Demonstrated experience in understanding and managing care plans and clinical assessments
  • Solid understanding of My Aged Care and Home Care Packages or similar commonwealth funding package models
  • Excellent people management, technology, and communication skills
  • Self-motivated individual with a practical and professional approach to case management
  • Current Police Check, CPR Certificate, and two (2) COVID Vaccinations (required)
