The Role

This key position involves overseeing quality operational services, maintaining efficiency, cost-effectiveness, safety, and meeting compliance requirements. As an expert care coordinator, your primary responsibilities include:

  • Developing and implementing comprehensive care plans in consultation with clients, their families, and healthcare professionals
  • Providing effective guidance and support to community support professionals to ensure seamless service delivery
  • Collaborating with our registered nurse and service team to facilitate, develop, and shape service outcomes
  • Building lasting relationships with NDIS customers through continuous communication and stakeholder engagement

Requirements

To be successful in this role, you will possess:

  • Demonstrated knowledge of NDIS practice standards and regulations
  • Excellent leadership skills to motivate and develop staff
  • High level of computer literacy with experience using information management systems
  • A passion for delivering high-quality care and support services
  • Understanding and experience in risk, complaints, and incident management
