About the Role
The Program Leader plays a pivotal role in ensuring that programs and initiatives are person-centred, working alongside Program Workers to model behaviours and practices that align with a wellness and reablement approach.

Key Responsibilities
• Lead and support a team in planning and implementing activities that foster client wellbeing and reablement
• Manage referrals, conduct assessments, and put in place interim support strategies where needed
• Prepare programs, organise necessary equipment and resources to undertake activities
• Respond to feedback from clients and their families by working with the Team Leader to develop programs and new initiatives that are appealing to older people across a wide age spectrum
• Monitor client health and wellbeing, reporting any changes in status to the primary carer and/or family member
• Accurately report on statistical client-related data in a client management system, with the ability to monitor progress and achieve funding targets
• Liaise with other service delivery teams internally and externally to respond to service needs of our participants

Requirements
• Tertiary qualification in human services areas, such as allied health, disability, or related discipline (desirable)
• Aged Care Certificate or lesser formal qualifications with relevant experience and work skills
• Experience working with people from diverse backgrounds, including those with memory loss, disability, cultural diversity, and social isolation
• Level 2 First Aid Certificate
• Excellent communication skills
• Advanced skills in time management
• Demonstrated experience in program development
• Established computer skills and willingness to learn specific software programs
• Current Victorian Driver's Licence and willingness to drive clients in a 12-seater minibus
• Bi-lingual skills and ability to speak a community language (highly desirable)
