We are seeking a highly skilled Paediatric Care Coordinator to join our team at Kalwun Health Service – Miami Clinic. This is a permanent part-time position, working five days per fortnight.
The successful candidate will be responsible for coordinating care for children from early childhood through to teenage years and their families. This role will entail both coordination as well as direct clinical care.
- Coordinate care for the paediatrician and other paediatric specialist services
- Deliver clinical services including assessment of the infant and child
- Initiate and participate in case management and review meetings with both internal and external stakeholders
- Develop effective linkages with external services and agencies
- Ensure children have a current completed 715 health check
- Collaborate with internal services such as Allied Health services
