We are seeking a skilled Community Support Specialist to join our team. As a key member of our team, you will play a crucial role in the delivery of our services.
Key Responsibilities:
- Process new client referrals and coordinate initial onboarding or further referrals to other supports
- Undertake client assessment against eligibility requirements, ensuring a person-centred approach to care needs
- Develop reports to support program management and adherence to program guidelines and contract requirements
- Facilitate and manage a partnership approach with families and supporters
- Collaborate with key business partners and external stakeholders to ensure consistency in program delivery and building a strong service culture
- Manage, monitor and report on incidents and complaints
Requirements:
- Relevant qualification in Community Services, Disability, Aged Care or Business
- Experience in disability, aged care or community services
- Administration skills, including experience in use of information systems and data management
- Strong verbal communication and relationship building skills
- Ability to manage competing priorities and deliver quality outcomes within given timeframes
- Strong computer literacy and ability to analyse, review and present data
