Job Description:

We are seeking an experienced Senior Care Coordinator to oversee the care coordination process for our clients in the Self-Managed Plus program.

Key Responsibilities:
  1. Engage with clients to understand their unique needs and preferences.
  2. Help them plan services like personal care, cleaning, and allied health support.
  3. Coordinate communication between clients, families, and healthcare providers.
  4. Support clients in managing their budgets confidently.
  5. Collaborate with internal teams and subcontracted providers to ensure seamless service delivery.
  6. Conduct regular check-ins to ensure client satisfaction and identify areas for improvement.
Required Skills and Qualifications:
  • Confidence in juggling multiple priorities and staying organized.
  • Effective and sensitive communication skills.
  • Comfortable using systems and learning new technology.
  • Previous experience in customer service, administration, or coordination roles.
  • Curiosity, kindness, and a willingness to take initiative.
