Case Manager

We are seeking a highly skilled and compassionate Case Manager to join our team at Wintringham. As a Case Manager, you will play a vital role in supporting older people who are at risk or have experienced homelessness with a range of high-quality care, housing, and support services.
Key Responsibilities- Assist clients in maintaining their independence and lifestyle of their choice by providing a high level of care and support.
- Help address issues of social isolation for program clients and assist clients to maintain or improve their accommodation.
- Undertake initial and ongoing assessments for new clients to ensure their needs are met.
- Develop and implement care plans and clients' schedules to ensure effective service delivery.
- Participate in care reviews with managers, clients, and carers to ensure the best possible outcomes.
- Effectively manage services provided within budgetary constraints to ensure efficient use of resources.
- Work with an in-house rostering team to ensure clients receive the care and supports they require according to their funded level of package.
- An approved tertiary qualification in Social Work, Allied Health, or Nursing.
- Experience working in mental health (desirable).
- Previous experience as a Case Manager (would be highly regarded).
- High level computer, documentation, and budgeting skills (mandatory).
- Current drivers' licence with a reliable vehicle suitable for transporting clients.
- Knowledge of housing and support issues facing older homeless people who may have a physical and/or mental disability.
- Skills in assessing clients' psychosocial, care, support, health, and housing needs.
