Senior Speech Pathologist

We are seeking an experienced Speech Pathologist to join the Statewide Care of the Older Person and Community Transition (CO-ACT) program. As a Senior Speech Pathologist, you will provide high-quality care to patients with complex swallowing, communication, and cognitive disorders, supporting improved functional outcomes and discharge planning.
You will collaborate with an interdisciplinary team, contributing to the development of Models of Care and patient pathways, while also working closely with external service providers to ensure smooth patient transitions.
Key responsibilities include:
- Providing evidence-based, consumer-centred care in speech pathology
- Collaborating with a multidisciplinary team to develop Models of Care and patient pathways
- Working closely with external service providers to ensure smooth patient transitions
We are looking for a skilled and dedicated professional with the following key attributes:
- Proven ability in conducting assessments, goal setting, and delivering evidence-based care in speech pathology
- Strong collaboration skills to work effectively within a multidisciplinary team
- A minimum of three years' clinical experience in relevant settings, with expertise in managing dysphagia and cognitive/communication disorders
