Job Opportunity: Home Health Physical Therapist

About the Role

We are seeking a skilled physical therapist to join our team and deliver exceptional care to patients in their own homes and communities. The ideal candidate will have a solid background in home health care, a deep understanding of state and federal regulations, and a genuine passion for helping patients live their best lives.

Responsibilities
  • Conduct comprehensive physical therapy evaluations and create personalized treatment plans to help patients achieve their goals.
  • Provide direct physical therapy treatments and interventions to improve patient mobility, balance, strength, and functional ability.
  • Teach patients and their families how to properly perform exercises and activities to promote recovery and improve mobility.
  • Document patient progress, including changes in condition, treatment plan, and goals, and communicate updates to other healthcare professionals.
  • C coordinate care with other medical professionals to ensure the most effective treatment is being delivered.
Requirements
  • Graduate from an accredited physical therapy program with a valid Physical Therapy license in the state of Florida.
  • Minimum of one year of experience in physical therapy, preferably in a home health care setting.
  • Demonstrated ability to create effective physical therapy treatment plans that promote patient recovery.
