Requirements:
  1. Bachelor's degree in a related field (Social Work, Nursing, Healthcare Administration) or equivalent experience.
  2. Previous experience in care coordination or a related role in the healthcare industry.
  3. Strong organizational, communication, and interpersonal skills.
  4. Knowledge of home care regulations and industry best practices.
  5. Ability to handle and resolve conflicts effectively.
  6. Proficient in computer applications, including Microsoft Office and care management software.
  7. CPR and First Aid certification preferred.
  8. Passion for providing outstanding care to clients.
Preferred Skills:
  1. Clinical Expertise: Proven experience in providing direct client care and ensuring safety.
  2. Ability: Ability to develop, implement, and evaluate organizational and client-focused documentation in the system.
  3. Government Funding: Good understanding of the government funding environment for aged care and NDIS (not compulsory but preferable).
  4. Hospital Discharge: Good understanding of the hospital discharge environment.
  5. Health Assessments: Good understanding of health assessments and developing care plans to meet clients' care needs.
  6. Multitasking and Time Management: Ability to multitask and manage time effectively.
  7. Communication: Excellent verbal and written communication skills.
  8. Computer Skills: Intermediate to advanced computer skills, including familiarity with CRM systems.
