We are seeking a highly skilled and motivated Nurse Specialist to join our clinical team at Subee Pty. As a key member of our team, you will play a vital role in providing high-quality support and care to our clients.
Key Responsibilities- Develop and Implement Care Plans: Contribute to the formulation of care plans and clinical pathways that address individual client needs and enhance independence, abilities, and quality of life.
- Provide Personal Care: Assist clients with daily living activities, including showering, dressing, and personal grooming, with a focus on those requiring extra help due to specific physical problems or frailty.
- Health Education and Promotion: Contribute to the health education of patients to maintain and promote health, and support clients in managing their health and well-being.
- Medication Management: Assist with the administration of medication in accordance with Subee's Medication Management Policy and Procedure.
- Transportation and Support: Transport clients to and from home and day programs, and other activities and appointments, and provide support and assistance as needed.
- Team Collaboration: Work collaboratively with other healthcare professionals to ensure comprehensive care and support for clients.
- Qualifications: Bachelor of Nursing or equivalent, and current registration with the Australian Health Practitioner Regulation Agency (APHRA).
- Experience: Knowledge and experience of the community, aged care, and disability sectors.
- Skills: Strong communication and interpersonal skills, with the ability to work effectively with clients, families, and healthcare professionals.
