Job Overview

This is a rewarding community nursing role offering fixed-term contracts for experienced nurses to provide care across various funding streams.

Key Responsibilities
  • Deliver high-quality nursing care to clients with diverse needs including CHSP, wound care, and palliative care.
  • Conduct comprehensive assessments and develop personalized care plans.
  • Coordinate care and arrange referrals to relevant services.
  • Work flexibly across 7 days per week and participate in an on-call roster.
Essential Requirements
  • Minimum 3 years acute nursing experience and at least one year's experience in District Nursing or comparable Community Nursing or Emergency experience.
  • Current AHPRA registration as a Registered Nurse.
  • Demonstrated ability to conduct patient assessments and develop goal-based care plans.
  • Experience in Palliative Care Nursing is highly regarded with relevant qualifications an advantage.
  • Postgraduate qualification in Palliative Care Wound Care or High Acuity Care or a commitment to pursuing further studies.
