Job Overview

We are seeking an experienced and dedicated individual to fill the role of Assessment Officer, who will be responsible for conducting comprehensive assessments and developing personalized care plans to support carers.

The successful candidate will possess excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders, including carers, healthcare professionals, and community services providers.

Key Responsibilities:

  • Conduct thorough assessments of carers' needs and develop goal-directed care plans
  • Facilitate access to respite and other support services, including the purchase of services when appropriate
  • Provide coaching and support to individual carers, promoting their independence and self-care
  • Collaborate with healthcare professionals and community service providers to ensure seamless service delivery

Required Skills and Qualifications:

  • Relevant qualifications in a related field, such as social work or psychology
  • Proven experience in assessment and care planning, preferably in a healthcare or social services setting
  • Excellent communication and interpersonal skills
  • Ability to work effectively in a team environment and collaborate with diverse stakeholders
