About the Role

We are seeking a highly skilled Case Worker to join our team. The successful candidate will be responsible for providing support and guidance to individuals experiencing mental health issues, with a focus on delivering a stepped care approach to mental health therapy.

Key Responsibilities
  • Provide direct support to clients in their homes or community settings, as well as facilitating group programs and workshops.
  • Evaluate the effectiveness of interventions and modify safety, support, and recovery plans as necessary.
  • Develop and implement individualised plans to meet the unique needs of each client.
  • Foster strong relationships with community partners and stakeholders to promote mental health services and resources.
Required Skills and Qualifications
  • Diploma or degree in Mental Health, Community Services, Counselling, or Social Work.
  • 1-2 years' experience in a mental health-related service, with a focus on case management and support.
  • Demonstrated knowledge and understanding of working with vulnerable, disadvantaged, and complex needs clients.
  • Ability to communicate effectively with a range of stakeholders, including clients, families, and community partners.
  • Computer literacy and ability to work with databases.
  • National Police Certificate and WA Driver's Licence required.
