Community Continuity Specialist

The primary objective of this role is to ensure a seamless transition of patients from correctional centres to the community, thereby improving continuity of care.
To achieve this goal, the successful candidate will be responsible for:
- Actively linking patients with appropriate services that cater to their unique needs.
- Utilizing an extensive network of contacts in both correctional and community settings.
This position requires a deep understanding of drug and alcohol treatment approaches and the ability to collaborate effectively with community-based networks.
