Prevention and Population Health Nurse Opportunity
Katherine West Health Board is seeking a skilled Prevention and Population Health Nurse to join their team in the Katherine West region of the Northern Territory. This role is ideal for a nurse with substantial clinical experience who is passionate about improving the health and wellbeing of remote communities.
About the Role
The Prevention and Population Health Nurse will work closely with community-based Primary Health Care Teams to provide, plan, and manage care for clients with rheumatic heart disease. This will involve health promotion and community education, data collection and monitoring, health hardware, clinical services, training, and research in the area of treatment and prevention of RHD.
Key Responsibilities
- Support the Prevention and Population Health Coordinator with the maintenance and review of the RHD program across the Katherine West region.
- Support clients and their families with self-management of RHD.
- Assist with the development and implementation of resources and strategies to increase adherence to secondary prophylactic treatment.
- Provide clinical leadership to health centre staff in relation to identification and treatment of rheumatic fever, rheumatic heart disease, post-strep glomerulonephritis, and throat and skin infections identified in the population within the Katherine West region.
- Assist with other preventative and population health activities such as immunisations, trachoma, and anaemia treatment.
About You
- Understanding of issues affecting Aboriginal health, principles of primary health care, and relevant legislation governing work practice.
- Understanding of the CARPA and RHD guidelines and the evidence supporting these guidelines and a willingness to demonstrate leadership in the use of these guidelines.
- Ability to communicate and work effectively with Aboriginal people and/or in a cross-cultural setting.
- Ability to show leadership in the area of rheumatic heart disease to the multidisciplinary team through guidance, education, training, and support.
- Commitment to own professional development and sharing knowledge and skills.
- Understanding of the principles of Quality Improvement processes and their application in the health care setting.
- Clinical duties within your skillset as directed by the General Manager Health Services.
