We are seeking an experienced and dynamic Registered Nurse to join our Hospital in the Home (HITH) Service team. As a key member of our multidisciplinary team, you will play a vital role in providing high-quality patient care and contributing to the growth and development of our service.
Key Responsibilities- Provide direct acute level care for a diverse clinical cohort across medical, surgical, pre/post-operative, geriatric/GEM, and rehabilitation specialties.
- Work closely with the multi-disciplinary team and Nurse Coordinator to coordinate patient care and problem-solve issues proactively to ensure an exceptional patient experience.
- Develop and implement effective care plans that meet the unique needs of each patient.
- Collaborate with other healthcare professionals to ensure seamless care transitions and optimal patient outcomes.
- Contribute to the development and implementation of service improvement initiatives.
- Current registration as a Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA).
- Strong clinical skills and experience in a hospital setting.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Ability to work autonomously and as part of a team, with a strong focus on patient-centered care.
- Current Police Record Check and Working with Children Check.
- Current COVID-19 certificate, proof of current influenza vaccination, and other vaccination status consistent with public health orders.
