The role of the Palliative Care Specialist involves working closely with carers to provide intake, assessment, and planning services for those accessing support provided by the Carer Gateway across Queensland.
This specialist will focus on carers and care recipients who have a palliative diagnosis, assisting in accessing appropriate services. The objective is to work with carers over the phone to access information, support, and referrals relevant to their caring role.
Responsibilities include undertaking intake, assessment, and planning services, working with carers to access information and support, and providing customer service ensuring carers are listened to with patience and non-judgement. Support will also be given to carers navigating and accessing palliative services, including assistance with Advance Health Directives.
Key Responsibilities:
- Undertake intake, assessment, and planning services for carers accessing support provided by the Carer Gateway across Queensland.
- Work with carers over the phone to access information, support, and referrals relevant to their caring role.
- Provide a high level of customer service, ensuring carers are listened to with patience and non-judgement.
- Assist carers in navigating and accessing palliative services.
- Support Carers and/or Care Recipients who have completed an Advance Health Directive.
Qualifications and Experience:
- Qualifications and/or experience working in community services, allied health or carer/family peer positions.
- Certificate of Palliative Care, Grief, and bereavement, and/or other palliative care training courses.
