The Physiotherapist - Home Care Specialist will be responsible for conducting comprehensive assessments and providing individualized therapy services to seniors in their homes, as well as group activities within the community.
Key Responsibilities- Conduct thorough assessments to identify clients' functional capacity, strengths, and lifestyle goals
- Develop and implement personalized therapy plans to promote healthy aging and quality of life
- Provide equipment prescription and recommendations for client care
- Regularly review and evaluate client therapy plans to ensure goals are achieved
- Maintain accurate and timely documentation in accordance with organizational policies and procedures
- Bachelor of Science (Physiotherapy) degree
- Current registration with Australian Health Practitioner Regulation Agency (AHPRA)
- Successful completion of a physical assessment and reference checks
- National Police Clearance (or willing to obtain within 6 months validity)
- Vaccination certificate for COVID and Current flu immunizations
- Current and valid motor vehicle Class C driver's licence
