
Senior Case Manager

We are seeking an experienced and passionate professional to lead our Home Care Package team. In this key role, you will oversee the coordination and delivery of high-quality client-centered care services.
Key Responsibilities:- Lead and coordinate the Home Care Package program, working in conjunction with senior management to ensure effective service delivery.
- Develop and implement a case management model of care, monitoring client outcomes and service effectiveness.
- Maintain efficient package expenditure within budget and regulatory guidelines.
- Contribute to business growth by driving Home Care Packages forward.
- Identify and implement service improvements to enhance care quality and competitiveness.
- Built strong relationships with stakeholders, including government agencies, healthcare providers, and community organizations.
- Ensure compliance with data collection requirements and funding targets.
- Manage compliments, complaints, and feedback, striving for continuous improvement.
- Minimum 3 years' experience in aged care, home care, or a related field.
- Solid knowledge of the Home Care and/or Aged Care industry, including relevant regulations and accreditation processes.
- Excellent communication skills with the ability to build rapport with internal and external stakeholders.
- Experience in budgeting and fund management related to Home Care Packages.
- Willingness to travel both intra and interstate.
- Availability to participate in after-hours on-call duties.
