Physiotherapist - Hospital Admission Risk Program

We are seeking a highly skilled and motivated Physiotherapist to join our team in the Hospital Admission Risk Program (HARP) at Epping Private Hospital. As a key member of the multidisciplinary team, you will be responsible for providing high-quality, evidence-based physiotherapy services to patients with chronic diseases or complex health needs in a centre-based setting, in their home or residential care.
The ideal candidate will have significant clinical experience in delivering physiotherapy services for patients with multiple comorbidities, complex needs, and chronic conditions. You will also possess excellent leadership skills, initiative in developing and promoting clinical care, and the ability to work effectively in a team environment.
About the Role- This is a fixed-term part-time position (46 hours per fortnight) commencing August 2025 until June 2026 to cover maternity leave.
- Deliver high-quality, evidence-based physiotherapy services to patients with chronic diseases or complex health needs.
- Work collaboratively with the multidisciplinary team to develop and implement individualized care plans.
- Conduct thorough assessments and develop treatment plans that address short-term mobility goals and optimize safety and function.
- Provide education and support to patients, families, and caregivers on managing chronic conditions and improving functional abilities.
- Maintain accurate records and communicate effectively with the healthcare team.
- A degree in Physiotherapy from an accredited institution.
- Current registration with the Australian Health Practitioner Regulation Agency (AHPRA).
- Significant clinical experience in delivering physiotherapy services for patients with multiple comorbidities, complex needs, and chronic conditions.
