Job Title:
Occupational Therapist - Home Care Specialist

About the Role

We are seeking an experienced Occupational Therapist to join our team and deliver high-quality home care services to clients in their own homes across Sydney.

The successful candidate will have a strong background in community care, aged care or similar field, with excellent clinical reasoning, independence and time management skills.

  • Manage a caseload of clients aged 75+ in their homes across Sydney.
  • Focus on home modifications, assessments, recommendations and reviews.
  • Work from home model with no office attendance required.

Key Responsibilities:

Assess and provide interventions for clients living with disabilities.

Develop and implement individualized plans to maximize clients' level of function, capacity and social engagement.

  • Conduct home assessments and recommend necessary equipment and modifications.
  • Collaborate with multidisciplinary teams to ensure comprehensive care planning.
  • Provide timely documentation and report writing.
  • Manage a varied caseload and meet professional expectations.

Requirements:

To be successful in this role, you will need:

  • Relevant tertiary qualifications in occupational therapy.
  • AHPRA registration (unrestricted).
  • Working with Children Check (WWCC or Blue Card).
  • NDIS Worker Screening Check Police Check & International Police Check (if applicable).
  • Driver's license and access to a reliable vehicle.

You will also need to be self-motivated, able to work independently and have excellent communication skills.
