Clinical Pharmacist

About the Role
We are seeking a highly skilled Clinical Pharmacist to join our team at Mulgrave Private Hospital in VIC. As a Clinical Pharmacist, you will play a vital role in providing exceptional care to patients, from admission to discharge.
Key Responsibilities
- Prioritise patients' needs and admission reviews from referring doctors
- Complete Best Possible Medication History for high-risk patients or as directed by ED doctors
- Conduct medication chart reviews and medication reconciliation
- Dispense, order and supply medication and pharmacy products
- Coordinate patient medication requirements for inpatient stay, day infusions and discharge
- Deliver a high level of clinical pharmacy services to our patients in accordance with professional, legislative and ethical standards.
- Develop and maintain professional relationships with key stakeholders to ensure excellent customer service to clients and patients
- Support patients, carers, hospital staff and associated employees
- Educate and advise staff and customers on health promotion, disease prevention, drug selection and the proper use of medicines
About You
We are looking for a passionate individual who has strong organisational skills and high attention to detail. This is a fast-paced and high-risk environment; therefore, you will have the ability to work under pressure while delivering a high standard of patient care with sensitivity, empathy and professionalism.
Requirements
- Current Pharmacist registration with AHPRA
- Relevant hospital pharmacy experience
- Excellent verbal, written and interpersonal communication skills
- Highly driven and enthusiastic
- High level of time management, planning and organisation ability
- Focused on delivering a high standard of customer service
