Personalized Care Solutions for Elderly Individuals

Are you a qualified nurse seeking a role that combines clinical knowledge with a passion for care?

This in-office position allows you to apply your skills meaningfully, supporting older Australians to live independently and well.

Our team is looking for a Healthcare Professional who can conduct personalized assessments, develop clear care plans, and recommend tailored services to empower clients to maintain independence.

Key Responsibilities:

  • Conduct thorough assessments to identify individual needs and create goal-oriented care plans.
  • Collaborate with clients and their representatives to develop clear, actionable goals.
  • Recommend services and supports that promote client autonomy and meaningful outcomes.
  • Identify potential health and safety risks through targeted questioning and medical history reviews.
  • Make appropriate referrals for clients presenting with high-impact or high-prevalence concerns.

Requirements:

  • Enrolled Nurses, Registered Nurses, or individuals with a Certificate III in Individual Support (Aged Care).
  • Experience in aged care, Home Care Packages (HCP), NDIS, or care planning is highly desirable.
  • Excellent communication and interpersonal skills, with the ability to work independently and collaboratively.
  • Digital confidence with Microsoft 365 tools and CRM systems.
  • Proactive and self-motivated, with a strong focus on delivering exceptional client experiences.
