Palliative Care Social Worker

We are seeking an experienced Social Worker to join our Palliative Care team in Central Adelaide. As a key member of our inter-disciplinary team, you will play a vital role in supporting patients with complex psychosocial needs and their families during challenging times.
Your expertise will be utilised to provide counselling and bereavement support, ensuring that each patient receives holistic care tailored to their unique circumstances. You will also contribute to planning, implementing, evaluating, and reviewing a comprehensive range of services designed to meet the psychosocial needs of our community.
About YouTo be successful in this role, you will possess:
- Qualifications: A relevant degree or equivalent qualification, making you eligible for full membership of the Australian Association of Social Workers.
- Skills & Abilities: Highly effective communication skills (verbal and written), adaptability to change, counselling and psycho-social assessment skills, and the ability to work in interdisciplinary settings.
- Experience: Significant experience in health settings, including providing counselling, making complex clinical decisions, and collaborating with diverse professionals and communities.
- Knowledge: A deep understanding of social work theories, psycho-social intervention strategies, grief theory, and culturally sensitive practices, including the health needs of Aboriginal and Torres Strait Islander peoples.
