Job Opportunity:

We are seeking a highly skilled and dedicated care coordinator to join our team in the transition care program.

About the Role:

The successful applicant will be responsible for providing support, care planning and coordination to clients and their carers in their own homes and/or in a bed-based setting. This will involve developing collaborative relationships across healthcare services and community programs to provide high-quality coordination, support and timely management of TCP clients.

Key Responsibilities:

  • Act as the central point of contact for all involved in the care of the client and utilize strong communication skills to liaise with the multi-disciplinary team and clients/families.
  • Provide comprehensive care planning and coordination to ensure seamless transitions between healthcare settings.

Requirements:

The ideal candidate will have dependant on award classification, COVID-19 vaccination and relevant experience in a similar role.
