Senior Case Manager Position

We are seeking a dedicated and compassionate Senior Case Manager to join our team. As a Senior Case Manager, you will play a critical role in providing holistic support and service coordination to individuals with acquired brain injury (ABI).

Your primary responsibility will be to coordinate services and supports for people with ABI, ensuring that they receive the necessary care and resources to achieve their goals.

You will work closely with funding bodies, healthcare providers, and other stakeholders to develop and implement personalized plans that meet the unique needs of each individual.

In this rewarding role, you will have the opportunity to make a meaningful difference in the lives of people with ABI and their families.

Key Responsibilities:

  • Coordinate services and supports for people with ABI
  • Provide options and alternatives to support the goals of the individual
  • Navigate complex service systems such as Housing, Centrelink, NDIS, Legal systems
  • SUPPORT AND LIAISE WITH FAMILIES AS APPROPRIATE
  • FACILITATE CASE CONFERENCES WHEN REQUIRED
  • E NSURE APPROPRIATE SAFEGUARDING REPORTING PRACTICES ARE UPHOLDED WHERE RISK TO CLIENTS IS INDICATED

Requirements:

  • Experience in case management or a related field
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
  • High level of organization and time management skills
  • Ability to navigate complex service systems
