About the Job

We are seeking a dedicated Home Care Support Specialist to join our team. The successful candidate will provide personalized support and assistance to our clients in their own homes, enabling them to maintain their independence and quality of life.

This is a rewarding opportunity for individuals who are passionate about making a positive impact in the lives of others. As a Home Care Support Specialist, you will work closely with our clients to understand their unique needs and develop tailored care plans to meet those needs.

You will assist with daily living activities such as personal grooming, medication management, transportation, social inclusion, and domestic duties. Your role will also involve maintaining accurate records and reporting any changes or concerns to our team.

Key Responsibilities:
  • Provide personalized support and assistance to clients in their own homes
  • Assist with daily living activities including personal grooming, medication management, transportation, social inclusion, and domestic duties
  • Maintain accurate records and report any changes or concerns to the team
  • Work collaboratively with clients and other healthcare professionals to develop and implement care plans
Requirements:
  • Certificate III in Individual Support, Aged Care, Home & Community Care, or Disability Services
  • HLTHPS006 - Assist Clients with Medication (or commitment to obtain)
  • HLTAAP001 - Recognize Healthy Body Systems (or commitment to obtain)
  • Current National Police Certificate
  • Current Drivers Licence and own reliable vehicle
